I acknowledge by signing this release form that I have been given the full opportunity to ask any and all questions I might have about obtaining a tattoo from Body of Art Tattoos & Piercings. I acknowledge that all my questions have been answered to my full and total satisfaction. I specifically acknowledge that I have been advised of the facts and risks of getting a tattoo.

Tattoo deposits are non-refundable and are required to secure your appointment date and time. The deposit compensates the artist for reserving your appointment, preparing artwork, communicating about the design, and turning away other potential bookings for that time slot.

A deposit may only be refunded if the tattoo cannot be completed due to circumstances that are specifically the responsibility of the artist, such as the artist canceling the appointment and being unable to reschedule or complete the agreed-upon service. Cancellations, no-shows, changes of mind, or circumstances on the client’s behalf do not qualify for a refund. Ignore
A client must be a minimum of 18 years of age and shall present (at the time of tattooing or body piercing) a valid, government issued, positive identification card including, but not limited to, a driver's license, passport, or military identification. The identification must contain a photograph of the individual and a printed date of birth.
